In order to file a claim against the United States of America, each claimant must fill out, print, and submit a Form SF-95 to the Federal Emergency Management Agency (FEMA).  You MUST call our office at (225) 383-4701 and complete an intake screening.   Once you have spoken with a representative in our call center, please complete the blank sections on the Form SF-05 following the directions provided. Please mail the completed form to Federal Emergency Management Agency (FEMA), Office of the Director, 500 C Street SW, Washington, DC   20472.

PLEASE NOTE: A separate SF-95 must be completed for each adult and each minor in your household

FRIDAY, FEBRUARY 29, 2008.

After mailing your original Form SF-95 to FEMA, Dumas & Associates YOU MUST mail a completed form to our office at Post Office Box 1366, Baton Rouge, Louisiana  70821-1366.
